3 Easy Steps We Will Walk You Through

Gather Required
Documents

Before the appointment, you must gather and bring:

 

  • A valid passport (or another government-issued ID)

  • Previous childhood and adult vaccination records

Then, call or schedule an appointment online with CardiaCare Center NJ

  • Patients must verify they have the required documents before booking.

  • Call (973) 777-2440, or book online.

Attend the Medical Exam

The immigration medical exam includes:

  • A physical examination by CardiaCare's USCIS-designated civil surgeon

  • Review of vaccination history (boosters may be required)

  •  Lab tests (immunity panel)

* If lab results or vaccinations are missing, additional steps may be required before completion.

We'll contact you when your results are ready

  • Lab results will take 5–7 days to process

* If additional vaccines or tests are required, the patient must obtain them and bring proof back to the clinic.

Receive Sealed I-693 an Submit Form

Once all requirements are met, the doctor will complete Form I-693 and place it in a sealed envelope.


* DO NOT OPEN the sealed envelope—it must be submitted directly to USCIS or the patient’s attorney.

The patient will take the sealed and unopened I-693 form to their immigration attorney or submit it directly to USCIS as part of their adjustment of status application.

Standard Process Time: 2 weeks
* If additional tests are needed, more time may be required for chest X-rays, additional lab work, or vaccinations.

  • Is this exam for U.S. citizenship?
    No, this exam is for adjustment of status (Form I-693), not the citizenship test (Form N-400). If you need an evaluation for the citizenship process, consult a neurologist or psychiatrist for exemption forms.
  • Who needs to take the immigration medical exam?
    Most individuals applying for a green card (permanent residency) are required to complete Form I-693, Report of Medical Examination and Vaccination Record, as part of their application.

    No, a valid passport or government-issued ID is required.

    You can call our office at (973) 777-2440 or schedule online. Before booking, please ensure you have all the required documents.
  • What happens during the medical exam?
    The exam includes: A physical examination Lab tests (e.g., tuberculosis and gonorrhea screening) A review of your vaccination history (boosters may be required)
  • Do I need to get vaccinations before my exam?
    If you are missing required vaccinations, we will provide a prescription for you to get them at a pharmacy. You must bring proof of vaccination back to our office.
  • Will I get my results on the same day?
    No, lab results take 5-7 days to process. If additional tests or vaccines are needed, this may extend the timeline.
  • What happens after my exam is completed?
    Once all requirements are met, we will complete Form I-693 and place it in a sealed envelope. Do not open this envelope—it must be submitted directly to USCIS or your attorney.
  • Can I get a copy of my I-693 form?
    Yes, we can provide an extra copy for your records upon request. However, USCIS requires the sealed envelope for submission.
  • What if my test results are abnormal?
    If any test results require follow-up (e.g., positive TB test), additional steps may be necessary, such as a chest X-ray or further testing. We will guide you through the next steps.
  • What if I need to reschedule my appointment?
    Please call us at least 24 hours in advance to reschedule your appointment.
  • Do you provide translation services?
    We offer services in English and Spanish. Patients typically bring a translator if needed.
